About the opportunity:

The role of the Care Coordinator is to support participants to achieve their NDIS and person-centred goals through the promotion of independence and choice. We work with support workers, support coordinators, plan managers & allied health professionals to provide a multidisciplinary approach to helping our participants achieve these goals. As a Care Coordinator you are the primary contact for your participant in all support-related matters. It is the Care Coordinator’s responsibility to ensure there is minimal disruption to participants care and services and maintain a strong level of communication with all people involved.

Responsibilities include: 

  • Onboard new participants and carry out the induction procedure by educating participants on NDIS processes.
  • Maintain NDIS and high care documents on a regular basis and/or as needed.
  • Completing and maintaining participant risk assessments, support plans and person-centred plans.
  • Help support participants with complex & high care needs – Ensure support staff are trained appropriately.
  • Maintain relevant documentation and matching registers.
  • Serve as the primary point of contact for participants and support workers.
  • Support participants in decision-making and coordinate a program both, in the community and in the home.
  • Demonstrate excellent customer service skills when working with participants, support workers and other team members.
  • Perform administrative tasks relevant to your works.
  • Demonstrate excellent communication skills.
  • Undertake HR administrative duties as required, such as hiring, onboarding, and inducting new support workers and matching them suitably to participants.
  • Maintaining a monthly roster for participants and support workers to provide a strong continuity of care.
  • Generate weekly timesheets and assist the finance team in budgeting monthly.
  • Communicate sensitively, openly and effectively with Hi Five participants, their families/carers and/or advocates, substitute decision-makers and other staff including providing prompt, courteous and relevant responses to all enquiries including internal and external requests.

Desirable / Requirements:

  • Appropriate certification which is relevant to the disability industry
  • Knowledge of the NDIS & complex care participants
  • NDIS Worker Screening Check
  • Australians Driver’s License
  • Police Check
  • Working with Children’s Check
  • Proof of Vaccination
  • Available to start ASAP!

Knowledge & Experience:

  • Previous experience in the disability industry including working with complex needs participants.
  • Strong administrative & communication skills.
  • Ability to manage time efficiently in a fast-paced work environment.
